Based within Airedale/Wharfedale

Our Integrated Care Team provide a dedicated and trusted point of contact for individuals, carers and families to assist with the navigation of their care and support planning; providing advocacy on their behalf within a multi-disciplinary team and developing innovative ways of supporting people with complex care needs, to remain independent for longer and experience better health and wellbeing.

The Support Workers will work as part of a team, supporting individuals to improve their health and wellbeing, ensuring that they have the support required to live independently within their home and can maximise the resources available to them.

You will have experience of working in a community setting, preferably within the voluntary sector and be committed to using a person centred approach to meet identified needs. You should be well organised with great communication skills, used to effective team working, but also have the ability to work independently.

The nature of the post will require travel across the District. A full driving license, appropriate insurance cover and use of a car is essential.

Salary £16,500 per annum (pro rata)

Hours worked on a flexible rota Monday – Friday. Occasional evening or weekend work may be required.

5 weeks holiday plus bank holidays (pro rata)
