Support Worker

Fixed-term for 18 months, from October 2019

This post presents an exciting opportunity to make a difference by working as part of a small Age UK Bradford District team, that is closely linked to primary care, to deliver ‘hands on’ person centred support, during a discrete intervention period, to individuals aged 65+ based on the Personalised Integrated Care Programme developed by Age UK.

This post is funded as part of a £2.7m research programme (PROSPER) which aims to improve the quality of life for older people with frailty by giving them choice and control over decisions about their own health and well-being.

Ideally, you will have experience of supporting vulnerable individuals to achieve their personal goals through building a trusted relationship. You will be patient, but persistent and able to motivate individuals using a creative and flexible approach. You will provide continuity for individuals by linking closely with the Team. Working within the bounds of the co-produced action plan you will make sure things are not forgotten. But, you will be confident in raising any new considerations with the PROSPER Team.

The post will require travel and full driving license, appropriate insurance cover and use of a car is essential.

Salary £18,746 FTE, £9,373 Actual per annum
18.5 hours per week - occasional evening or weekend work may be required.
5 weeks holiday plus bank holidays (pro rata)
