The Bridge Project is looking for a resourceful, motivated individual to become a Self Care Champion. The Self Care Champion will work within a Community Partnership of 6 GP practices in South Bradford which serves a population of approximately 52,000 people. The post holder will work with a range of statutory and voluntary sector organisations to support and promote self-care through collaborative working within their community and locality.

The ideal candidate will have experience of working in a health care or voluntary sector setting and a broad knowledge of relevant health issues and self-care interventions.
