WE ARE ADVERTISING 4 POSTS. 2 X BRADFORD ROYAL INFIRMARY (1 X 37 HRS & 1 X 24 HRS) AND 2 X AIREDALE GENERAL HOSPITAL ( 1 X 37 HRS & 1 X 24 HRS) . HOURS INCLUDE 1 DAY PER WEEKEND.

The Carer Navigators will primarily work through the hospitals to support carers and families of people admitted to hospital with life changing conditions.  You will support carers and families to navigate the health and care systems and to make important decisions about arrangements post discharge.  You will have experience of casework, assessments/reviews, providing information, advice and basic advocacy as well as knowledge of welfare benefits, financial choices and the statutory sector.
