Our Community Connectors’ provide a service that through short term interventions links isolated people with community activities that improves their health and wellbeing.

The Thornbury Centre is looking for an energetic individual with people-focused skills, and previous experience of working in the health/ mental wellbeing sector, to play a vital role in our social prescribing service for patients in Bradford.

The post is part of a wider project, delivered by HALE on behalf of City and District CCG. HALE is based in Shipley and the post holder will work with colleagues from HALE and its other partner organisations.

Applicants must have a full valid driving licence and access to a vehicle which is fully insured for business purposes.

Part-time 14hrs per week Salary: £11.05 per hour

Funded until March 2020 with possible extension
