Bi Lingual Community Connector

A Community Connector provides short term interventions for people who are isolated, experience mental health problems or who have had a sudden crisis in their lives which results in their needing support to access community activities. You will work closely with GP Practices and health professionals taking referrals and ensuring a joined-up approach to health and social care

The successful candidate will have excellent interpersonal skills, alongside the ability to be able to successfully communicate with our BAME or Eastern European and Roma service users, both as a Community Connector and in outreach groups throughout Bradford. 

 

The post is part of a wider project, delivered by HALE on behalf of City and District CCG. HALE is based in Shipley and the post holder will work with colleagues from HALE and its other partner organisations.

 

Applicants must have a full valid driving licence and access to a vehicle which is fully insured for business purposes.

 

14hrs per week £11.05 per hour

 

Funded until March 2020 with possible extension
