Home from Hospital Co-ordinator - Craven Area based in the Craven Locality with frequent travel across the area

22 hours per week, Thursday, Friday, Monday with some weekend cover on a rota basis £21,601 (all salaries quoted for 37 hours)

You will develop relationships with appropriate hospital and community teams and directly provide short-term support (no personal care) to clients in their own homes after discharge, particularly in the North Craven area.

Joining an experienced team, you will be confident in working flexibly and independently in the community and within hospital settings. Ideally with an understanding of the issues that can arise post-discharge, you will have:

• Experience in assessing client needs and support planning
• Experience in supporting people in their own homes
• Confidence in liaising with hospital staff and community health & social care teams  
• Excellent people skills
• Experience of working within a hospital environment will be an advantage
