Based at Airedale Hospital, working 3/4 days a week Monday - Sunday on a rota basis between 10am-8pm

Age UK Bradford District is a local, independent charity with over 30 years experience supporting and working with older people in the Bradford District.

We are excited that this winter we will be working in partnership with Airedale General Hospital and Bradford Teaching Hospitals. As a key partner we will take a strength based approach to ensure that older people with frailty get the support that is right for them, in the right place at the right time.
We are looking to build a team of 5 Personal Support Navigators who will provide this support as part of the brand new multi- agency support team (MAST) – working alongside adult social care, clinical leads, VCS alcohol and peer support workers as well as the wider urgent care teams including the virtual ward.
You will be committed to using a person centred approach to ensure the personal aims and choices of people are properly represented within the multi-disciplinary team. You should have the ability to think innovatively and find creative solutions; involving other organisations and volunteers; being effective as a facilitator rather than a fixer.
Salary £20,108 per annum FTE
Actual salary £14,265.81
5 weeks holiday plus bank holidays (pro rata)
Fixed term contract to 31st May 2020
