Personal Support Navigator

Fixed Term Contract to 31st March 2021

Based within Airedale & Wharfedale

Our Integrated Care Team provide a dedicated and trusted point of contact for individuals, carers and families to assist with the navigation of their care and support planning; providing advocacy on their behalf and developing  strengths based ways of supporting people with complex care needs, to remain independent for longer and experience better health and wellbeing.

The Personal Support Navigator will ‘bridge the gap’ between the health and social care systems and individuals.

You will have experience of working in a community setting, preferably within the voluntary sector and be committed to using a person centred approach to ensure the personal aims and choices of people are properly represented.  You should have the ability to think innovatively and find creative solutions; involving other organisations and volunteers; being effective as a facilitator rather than a fixer.

The nature of the post will require travel across the District. A full driving license, appropriate insurance cover and use of a car is essential.

Salary £23,230 per annum FTE, actual salary £17,579

28 hours per week over 4 days Monday – Friday, although occasional evening or weekend work may be required

5 weeks holiday plus bank holidays pro rata
