Age UK Bradford District is recruiting for PROSPER and has a vacancy available for the role of Personal Independence Coordinator. This post presents an exciting opportunity to play a pivotal ‘hands on’ role in coordinating health and social care services with voluntary and community sector networks. Working as part of a small Age UK team that is closely linked to GPs and primary care teams, you will be delivering person-centred support, for a limited time, to individuals aged 65+ based on the Personalised Integrated Care Programme developed by Age UK. These posts are fixed-term until May 2022 and are funded as part of a £2.7m research programme (PROSPER) - #@ ProsperTrial - which aims to improve the quality of life for older people with frailty by giving them choice and control over decisions about their own health and well-being.
Agile working is supported, but as you will be visiting people in their own home, a full driving licence is required, together with business use insurance and access to a car.
Salary £11,250 per annum actual (£22,500 FTE)
5 weeks holiday plus bank holidays pro rata
