A team of researchers based at UCL have developed a new and innovative home-based health and well-being promotion service for older people who are starting to become frailer. The HomeHealth service is designed to deliver a tailored programme of support and advice to address the symptoms of mild frailty, through finding ways to enable the older person to go out regularly, manage things that are becoming more of a struggle, and help to plan ahead to enable them to keep doing what is important. 

The postholder will manage a caseload of older people with mild frailty, supporting them to identify and set goals, and developing together a tailored programme of support in relation to exercise, nutrition, socialising and/or mood.

Ideally you will have experience of working with the older population in community settings, together with experience of engagement and person-centred planning.

The role is subject to satisfactory references and DBS check, which will be arranged and paid for by Age UK Bradford District.

Agile working is supported, but there will be some travel to medical centres and Age UK hubs throughout the District. Therefore a full driving licence is required, together with business use insurance and access to a car.

Salary £10,634 per annum (21,268 FTE)

18.5 hours per week

5 weeks holiday plus bank holidays pro rata
