A team of researchers based at UCL have developed a new and innovative home-based health and well-being promotion service for older people who are starting to become frailer. The HomeHealth service is designed to deliver a tailored programme of support and advice to address the symptoms of mild frailty, through finding ways to enable the older person to go out regularly, manage things that are becoming more of a struggle, and help to plan ahead to enable them to keep doing what is important.
Working as part of our wider Integrated Care Team, the postholder will manage a caseload of older people with mild frailty, supporting them to identify and set goals, and developing together a tailored programme of support in relation to exercise, nutrition, socialising and/or mood.
Ideally you will have experience of working with the older population in community settings or a background in health and social care or the charity sector.
The role is subject to satisfactory references and DBS check, which will be arranged and paid for by Age UK Bradford District. The role is currently funded to 31st May 2022, with the possibility of extension beyond this date.
Agile working is supported, but there will be some travel to medical centres and Age UK hubs throughout the District. Therefore, a full driving licence is required, together with business use insurance and access to a car.
Salary £10,634 per annum (21,268 FTE)
18.5 hours per week
5 weeks holiday plus bank holidays pro rata
