Supported Discharge Co-ordinator (BRI)

Home from Hospital Supported Discharge Co-ordinator

Based in Shipley office and

25hrs per week Bradford Royal Infirmary

( £22,035 - £24,167) (for37 hours)

You will be working with A&E discharge teams to transport patients (over 60yrs old) being discharged from A&E and similar departments. You will work in partnership with Care@ Carers resource staff to support patients in their own homes, providing practical and emotional support to help them to maintain their independence. (No personal care)

You will have experience of working with people from diverse communities.

You will be confident in working flexibly and independently in the community.  Ideally with an understanding of the issues that can arise post-discharge, you will have:

  • Experience in assessing client needs and support planning
  • Experience in supporting people in their own homes
  • Confidence in liaising with hospital staff and community health & social care teams
