Community Carer Support Navigator

1 post covering Bradford West Locality area

37 hours per week

One year fixed term

 £23,133- £25,449 

This is an exciting one year project that has been created to assist carers to navigate the health and social care system and to engage with the support planning process. The postholder will work within Carers’ Resource but will work closely with community health teams and Age UK, Personal Support Navigators, in Bradford East/West area.

You will identify and address the needs of unpaid carers to promote, support and improve the mental, physical, emotional and economic well-being of the individual, so they can continue in their caring role. A strengths based, person centred approach is needed to empower individuals to identify, make and sustain positive lifestyle changes to improve their health and well being.  You will work in partnership with other external services in particular community health teams and statutory partners.

You will be based in our Shipley office, with some working from home, community and home visits

Salary scale quoted for 37 hours per week.
