Location: Outreach and office based at Bradford Royal Infirmary Site (City Hub)

Responsible to: Hub Coordinator

Salary: £24,898 (pro rata 37.5 hours full time equivalent)

Hours & Days of Work:    22.5 hours per week TUES-WEDS-THURS. 9:00am -5:00pm

Fixed term contract to 31/03/2023). May be extended subject to further funding.

OVERVIEW

As part of the Act as One Urgent and Emergency Care Programme funding been awarded by NHSE to support the local health and care partnership by establishing several localities based psychosocial hubs to provide health and wellbeing advice to local residents.

The Bridge Project are delivering one of the main Hubs based in the Yorkshire Cancer Support Annex building which is at the back of Bradford Teaching Hospital and is slightly different to the other Hubs as it will be a base for a number of VCS and statutory staff who provide non-clinical support to the hospital including but not limited to the BDFCT Core 24 mental health psychiatric liaison team and the VCS Multi-Agency-Support-Team (MAST). It will be used to pilot an integrated team model, and ‘streaming’ directly from the Emergency Department into a non-clinical, therapeutic safe space.

Purpose of Role

Working collaboratively with the Hub coordinators and across voluntary, social and health sector organisations in the Bradford area to scope, map and develop referral pathways between organisations including those delivering in the Network. Support clients to access appropriate services and support in the community to help meet their needs.
